No Doctor-Patient Relationship: Participation in a discovery call with one of our doctors does not establish a doctor-patient relationship. The purpose of this call is solely to determine if the doctor and the prospective patient are a good fit to work with one another. During the call, the doctor will endeavor to understand the prospective patient’s goals and health concerns in order to recommend the program that may be best suited to their needs. However, it is explicitly understood that any discussion during this call is for informational purposes only and should not be construed as medical advice, diagnosis, or treatment. No liability shall be assumed for any actions taken based on the information provided during this call. A formal doctor-patient relationship will only be established upon the execution of a separate written agreement and completion of the required intake process.

Non-Refundable Deposit: Upon the conclusion of the discovery call, if the prospective patient elects to proceed with one of our programs, a non-refundable deposit shall be required to reserve a spot on our doctor’s schedules. This deposit is a condition precedent to securing your appointment and shall not be refundable under any circumstances. By making this deposit, you acknowledge and agree that this payment secures your reservation and is forfeited if the appointment is canceled or rescheduled.

Confidentiality: All information disclosed during the discovery call shall be maintained in strict confidence in accordance with applicable privacy laws and regulations. We are committed to protecting your privacy and ensuring that any personal or medical information shared during the call is safeguarded and not disclosed to any unauthorized third parties without your explicit consent, except as required by law.

By participating in a discovery call, you acknowledge that you have read, understood, and agree to the terms of this disclaimer. You further agree that this disclaimer constitutes the entire agreement regarding the nature and purpose of the discovery call and supersedes any prior communications or understandings.
